I've always heard people say often, the holy spirit spoke to me, God said i should do this, Jesus said i should not do that
My question is is -

Have you heard him speak?
How do you hear him?
Like a voice in your head?
With your ears?

God does speak! The human is a tripartite being i.e man is made of body, soul and spirit. With your body you contact the physical realm, with your soul you contact the intellectual realm and with your spirit you contact the spiritual realm. God is Spirit. He speaks to our Spirit. He does speak with a "still small voice". The 1st issue is giving ones life to Christ(being born again) and we receive the Spirit of Christ (the Holy Spirit) immediately-Romans 8:9. The problem is that if our mind is not renewed we can't hear God speak. Romans 12:2. If our mind(soul) is not stayed on God's word(through reading, studying, meditating and memorizing the scripture) we can't here Him speak.

It's not a voice you just hear in your ear. Although rarely that may occur. I told you God speaks to our Spirit. You might not just understand this concept until you genuinely desire Him to speak to you and you start experiencing God speaking to you. I strongly suggest you get this book "The School of The Spirit" By Roberts Liardon. You need to develop your spirit to the point where God would speak to you. That's what that book would teach you with Biblical examples.

How distinguishable is the voice from my conscience or from my thinking

During the early part of my Christian life
I often assumed/mistook my conscience to be God of talking to me

Was a Christian for a while and never heard God speak to me

Just wanted to be certain I hadn't drowned out his voice or was expecting something too extrahuman to happen to me.

Looking at the bolded, would I be wrong to say that God is something that people create within their minds.

In a different way you are clearly confirming what most of us believe: That god is in the mind of the believer.
