They don't sell or issue out any form..if u possess the required qualification, simply go to Osun State command with ur application letter and credentials

application letter, photocopies of the following documents...Degree certificate,birth certificate, Local government certificate,primary school cert, NYSC discharge Cert, CV, Certificate or license of registration with professional bodies ( for Doctors, Lawyers, Engineers, etc) and two passport photographs.

Ok,my friends were like they demanded 2 copies of ur everything..that's ur written application,cv,credentials,4passport...2attached to each...I think dats all.. Though my zone was jus one copy of errtin n 2passports

samson212: From ikeja under bridge,take a bus to alausa secretariat,alight in front of the secretariat and take a walk to shangisha and cross to the other side of the express way then ask for the sss office very close to the NEPA office.....good luck

No. I addressed with the Abuja address and I was not ask to submite extra copies. Start with hand writen application first while arranging it then ur cvs,LGA state of origin or attestation from ur LGA chairman, birth cert, primary,wasc,degree, nysc and other provisional cert.
