How do I fill a direct deposit form with the U.S Treasury? How do I contact them? Do I have to call them or email them? To talk to them about the check I have now.

Because I was on the phone forever waiting on a queue...

walk into gtb to ask for a direct deposit form. After filling the form you send it to the particular parental that's in charge of your payment. For example mine is opm.hope this helps
