While trying to upload required document to Offa poly portal, the initial submitted details and passport photo have been deleted. And the form is to be refilled afresh. So, if you or someone you know apply to this poly, check your portal. You may need to refill your form and re-upload all your documents. It is possible that the site experienced a system crash thereby loosing essential data.
