A couple of illness could be responsible:

1) Vitamin C deficiency (scurvy)

2) Tuberculosis since its also associated with weight loss. There may not be associated cough but just clearing of throat is enough.

3) Gingivitis, etc.

He should be properly seen by a medical doctor ASAP.
