How does one know that it's been approved. . .

Someone told me the interviewer stamps it or signs it but I'm not sure they do that for everybody.

How does one know?

they usually tell you after the interview to wait for one hour to get your permanent visa stamped on your passport. If you are not asked to wait, you know outrightly nothing for u.

It happened to me at the Big House early February. Its no big deal, if you have your papers and have made the mandatory years - you will get it. Proove you are working, payslips, prove you have bank statements - thats it.

I am guessing you talking about the UK anyway.
