Although rare in humans, this indeed can happen. The cause is similar to dogs being stuck but not as severe to the male human. That is, the male can get loose if the female relaxes to end it. This is urban knowledge… But supposedly this usually occurs when the male and female may be virgins and the male is above average in penis head size that looks like a Trojan helmet shaped harpoon.

Apparently at some point the females vaginal muscles (perhaps due to nervousness or fear of getting caught in the act) go into a spasm that locks down and traps the penis head. Like a Boa Constrictor, the more the male tries to withdraw the tighter the involuntary muscle grip.
