Writing such letter is procedural for the visa application. I live here in the UK and had a Nigerian neighbour whose dad came over in March for kidney transplant and was accompanied by my neighbour's younger brother who was the donor and that was stated in the letter for the visa application.

Your use of "the boy" clearly shows someone who is a minor, as a 21 year old can't be addressed as a boy.

The test to know if the kidney is compatible or not is not done in Nigeria. You need to undergo the test first before the real transplant is done. Same thing goes for blood donor. You don't just donate blood just like that. A sample of your blood would first be taken for screening before you are asked to donate.
