I know Rauf aregbesola hospital very well, it is a primary health centre. They don't handle any case more than normal uncomplicated delivery. I wonder how this educated man registered his wife in only a primary health centre. He should have at least registered her in another private hospital. You don't even present early..she got there after she must have been in labour overnight. Forget the labour started by 5 am story. Moreso, the nurses were persistent early enough in informing him they couldnt handle it but he should go to igando but rather he kept making trouble wasting precious time. There are several good private hospital at egbeda but he kept telling them a hospital couldn't be that big without a doctor. The nurses under pressure were forced to do what they could not do thereby wasting more time. By the time he agreed to go he was asking for referral note in emergency. That there was no doctor on duty was administrative....a health centre is primarily run by CHEWS under the supervision of a doctor who may not be available 24 hrs except corpers. so he went to health centre blaming the nurses who rightly told him it was beyond them. I am not saying our health system is not rotten but some people make trouble at the wrong time. You will save your wife first then come back to make trouble. RIP and my condolences

I don't think I like this op at all. I'm trying to give him benefit of doubt here but...
Really, why did he waste vital time? From his own story, labour began at 5am and he was at the health centre by 5am. Hmmm. Also he says he was at ikotun general hospital by 6am. First is it possible for labour to happen under 1 hour? Plus all the other drama that took place? This woman should have been in a hospital when she started having labour pains. Not when she began bleeding.
And why take wife and baby to a hospital where there is no doctor? Are there no other hospitals around? Or he just wanted to avoid spending money at the cost of his wife and child? If truly everything happened in hour including the 25 mins the husband spent demanding for a note, then the hospital responded quickly. I would like to hear their side of the story including why a govt hospital does not have a dr available.
Government provides hospital services to care for all citizens. But some people prefer not to pay for those services. These kind make it difficult for those who are in genuine need to get help readily. I wish the wife eternal peace.

Classical "hyperphalooting" stuff when the basics have not been done.
Empathy from those on duty and doing the basics their training required cud have gone a long way.
They also appeared to have neglected the newborn while trying to now attend to the woman and the baby who most likely died from hypoxia too cud have been saved by basic suctioning and resuscitation.
Why not passing definitive judgement,its obvious that a lot that was supposed to have been done wasn't done, no matter what d final outcome wud have been.

We are Hippocrates Brothers,we don't wash our linen outside. Forget Basis, this in not a 4thMB Obgyn exam..cus he wouldn't have passed if he didn't know the management of PPH.

Ewoooooo! General hospitals in Lagos are nothing to write home about. May the souls of the faithful departed through the mercy of God, rest in peace, Amen. This is why I admire delta state governments in his Giant stride of free medical care for pregnant women . This situation can never happen in any general hospital in warri in particular. GOD BLESS UDUAGHAN AND GOD BLESS GOVERNOR IFEANYI OKOWA, for sustaining the projects. Take heart oga, it is well!

when we advise you, you won't hear. By the way, I am a doctor. Nigeria is so rotten so you need to understand how the system works for your own safety. Just this last week, we had a woman that was dying...she had bled so much and within a short period, we gave her 8 pints of blood without the husband depositing anything yet it is a private hospital. she went into a failure and we had to defribrillate her. But I will never forget something...the husband was so calm that he made our job so easy. By making trouble with the health people taking care if your family, you put them under pressure and as human beings they will make wrong choices. So, for your information, health centres are built to take normal uncomplicated delivery. This is nigeria and not UK. The idea is that rather than allow women patronise all these traditional birth homes and churches, let them deliver in a place close to them, cheap enough but at least they will be able to spot complications faster than traditional homes and direct them to secondary centres. Secondly, by making so much trouble, he put the workers under so much pressure they started attempting to do something just to please him while his wife was dying.

Yes oh! The two gentlemen u mentioned are Medical doctors. They were Commissioner for health at different times even without knowing that they will be governors someday
So they know what Maternal Mortality means. Delta health care system is very good.

Before I talk......RIP to the woman

I have a few questions for the case....

1.We do not know how long the woman was in labour before she got to the hospital. That is crucial. If she had been in labour for a prolonged period of time...something tells me that her condtion was a disaster waiting to happen.

Note that the water breaks when the cervix is fully dilated....meaning that by then the woman HAS been in labour for over ten hours. The man AND wife presented late to the hospital.....and something tells me that his wife did not go for ANC.

2.Why do we tell women to go for ANC?

Because disasters like this could happen?

We do not know the woman's ANC history at all. How long she attended clinic, whether she did ultrasound....so that we can catch any possible complications on time.

3.Yes the nurses sent her to Igando General....because this was an obstetric emergency....and I doubt a small clinic could handle post partum hemmorhage that bad.For that you need a specialist.....and that is why they sent them to Igando General. But our husband did not go...and vital time was wasted.

4.When she was taken there....there was no space. Husband made noise...they found a bed....but if she had lost lots of blood....the case was too late. She needed ICU plus transfusion of lots of blood And a hysterectomy. Am sure the lack of space hampered care....everyone is busy with lots of emergencies....and one more stretches the whole workload.

5.In my opinion, the woman died not because of nurse....but because of lack of adequate facilties, and lack of specialists.


NOW HERE IS WHAT SHOULD HAVE BEEN DONE.

1.Labour starts.

2.Woman is rushed to the nearest hospital by ambulance. LONG BEFORE THE WATER BREAKS.

3.Active management of labour starts with every change noted on a PARTOGRAM

4.Based on the changes....labour can be terminated medically or surgically (CS).

5.Post partum haemorrhage can be stopped by active measures.


The problem started long before the woman went into labour....and was prolonged by the fact that there was no adequately trained staff, and no adequately equipped hospitals.(Government .....una don hear? especially the APC governor of this state).....and poor health education.We also need more specialists....especially obstetricians.

If you are a woman reading this

1.Register for ANC as soon as you are pregnant.

2.Go to hospital when labour starts...do not wait for water to break.

3.Make sure there is an obstetrician on standby.

you just said it all, alot of people play with their life even though they know the kind of country we live, I'm pretty sure she has not been attending prenatal there and that they were not aware of any medical problems she might have, take adequate precaution for your health and lives and leave the rest to God, I drove myself to the hospital around that 4:30 to 5am after my water has broken, of course it was a private one and I lived around that axis, drove my myself back two says later with my beautiful dd.

So so sad. That health centre as well as other health centres within Lagos should be visited and equipped with both human and non-human resources.
Believe you me, the day my wife told me that she saw a woman during one of their ANCs in serious labour without being attended to (just because she left out a piece of the delivery materials) was the day I stopped her from using that health centre.
As much as it lies within your power, avoid them. If you can't, hope for a normal delivery.
If not, the doctors there don't even do CS. The patient will be referred to Igando.
Imagine, referring a woman in severe labour to Igando with the terrible traffic in Lagos now; your guess is as good as mine!!!!!

I wished this case had been handled differently by all the parties involved...I wailed loudly reading this post...it's d saddest narration I've read in recent times.
I wished the lady & d man had presented early enough at d onset of labour.
if only d nurses on duty at egbeda had been more emphatic...if only there was a doc on duty
if only the man had taken his wife to a nearby private hospital as soon as he was told that there was no doc and that they couldn't handle her case...maybe he had anticipated d bill & didn't have it
had there been an ambulance...had there been sufficient bed spaces and specialist attention at igando.
if only I had some super powers to help the poor baby and the mother.

she could easily have been my sister, my sil, my friend, my colleague...

I am not washing any linen of anyone outside.Rather,d way he made d case look hopeless was creating a wrong impression of lack of empathy for d lives lost.
As I said,d outcome might have been d same,but there was a wide gap in d chain of management that isn't properly accounted for.Professionally those met may not have done much to change d outcome,but d issue here is that they refused to do anything!From standard,maybe outcome changing mgt of mother and child,to referral,if only to protect themselves.That ain't good enough!!
As for d person I quoted,on a good day, I am sure he wud do very well faced with such odds.

There are 3 tiers of hospital: primary, secondary and tertiary. Most primary centers can't handle complicated cases, they often don't have doctors on duty 24hours except for the flagship centers, they don't have emergency and blood transfusion services. The man though educated is ignorant about how Heath services works. It's a sad event. People should know that complicated cases can't be handled at the primary health centers. Even some private hospitals without the necessary facilities can't handle complicated cases as well.

Well said. Most importantly, we must hear the other side of the story. This story doesn't add up at all. This case is not for PHCs,not even when there is a secondary center less than 10 mins away. The man wasted vital time due to no fault of his. I know that hospital and such a hospital would have been well equipped.
My solutions:
1. The government must employ more medical doctors and trained nurses
2. There must be at least one obstetrician per local government area serving all PHCs in that local government
3. Better referral system that includes ambulance services
4. More paramedics should be employed
5. Primary healthcare centers must start running an uninterrupted 24hours service.
6. The citizens must be enlightened to know when to present at primary centers or secondary centers.
7. Hospitals across board must be well equipped.
8. 24hours power supply

Rip to the mother and baby.
In this kind of situation,it is quite difficult to blame the health workers on ground as at that time.
"Above all,do no harm" (they felt the case was complicated,and they referred).With or without a letter,patient will be received at the secondary centre,all things being equal.however,the following will go far in reducing maternal mortality in the developing world;
1)Preconceptional and Antenatal care;many will not register,some will but will do very late(i have seen pregnant women registering for ANC at 8months)
2)women education and empowerment;some will not make prompt decision until their hubby is around neglecting the danger signs.
3)Recognizing the danger signs of pregnancy;bleeding,preterm contractions,preterm rupture of membrane,reduced fetal movement etc
4)Birth preparedness;be ready for the coming child in terms of funds,materials,transport,map out plan A,B,C in terms of good facilities where u could possibly give birth.
From the story above,she drained liquor at 5am,gave birth with the hour,this is not credible.At term,u deliver at 8-10cm cervical dilatation(cervix dilates at 1cm per hour).She must have been in labour for long before presenting hence complication readiness could not be addressed by the poor primary health centre.
However,they should have examined,determine the cause of the postpartum haemorrhage likely retained placenta,administer oxytocin(maybe it was not available and hubby was told to buy ),deliver placenta and then refer to where there is blood transfusion services(even tertiary centres at times have blood issues due to lack of electricity,seen this many times).If due to perineal tear from precipitous labour,suture tear,though some can be difficult to manage if its cervical tear.
There was no doctor on ground,well,its a primary health centre.
In this kind of situation,its difficult to lay blames but the lessons are there to learn.
RIP to the dead and may God console the husband(Amen)

See the cruel treatment melted out to a newborn and sending him/her back after barely spending 20mins on earth....

if they withdraw their liccense, the yeye ppl will go and open chemist with the drugs they have stolen from the hospital
